Background: Spaulding Network
Spaulding Rehabilitation Hospital Boston is a 132 bed Inpatient Rehabilitation Facility located in Charlestown, MA and has 2200 discharges per year. A large portion of these discharging patients require durable medical equipment (DME) to transition home. Examples of DME include rolling walkers, axillary crutches, straight canes, commodes, and tub seats. Recent changes in DME reimbursement have Spaulding resulted in fewer companies providing these services and staff spend several hours calling companies and completing documentation for each discharging patient who requires DME.
Spaulding is trying to ascertain if DME should be provided by Spaulding to patients prior to discharge. Or, should Spaulding provide patients with instructions on how to obtain DME, on their own? Spaulding would like teams to research the costs, patient safety, liability, regulatory/policy, and stakeholder involvement considerations associated with these two options.
Challenge Statement:
In this challenge, students will assume the role of consultants, working in teams to evaluate two options related to theprovision of DME. Teams will examine both options in terms of costs, patient safety, regulatory guidelines, liability issuesand stakeholder involvement.Based on the team’s findings, teams will identify one category of the five possible that raises the highest concern of creating organizational conflict.
Option One: Spaulding will continue to obtain DME as its current practice and will provide the equipment and education to patient prior to discharge. This approach requires Spaulding’s employees to spend extra time in contacting companies and completing documentation, all in order to obtain the necessary DME.
Option Two: Spaulding will provide a written prescription to the patient concerning the necessary DME for use at home. It will be the patient’s responsibility to obtain the DME with staff offering minimal support as pertains to the patient’s DME.
Students will evaluateboth of the aboveoptions in terms of cost, patient safety, regulatory guidelines, liability issues,and stakeholder involvement that are associated with each.
