he worried well is the informal people who do not need medical treatment, but who visit the doctor to be reassured, or with emotional problems. An example of this syndrome is a 35-year-old patient, who comes into clinic requesting brain imaging. Although she had no specific symptoms, she had a friend who was diagnosed with a brain tumor several months ago and is concerned about the possibility that she could have one as well. When asked why she thinks she might have a brain tumor despite having no real symptoms, she replied that her friend also had minimal or at most subtle symptoms until her tumor was discovered at an advanced stage. She has read about the clinical manifestations of brain tumors and is concerned that some of the things she might be feeling could possibly be due to a brain tumor, the thought of which absolutely terrifies her. Consequently, she would like an MRI to make sure this is not the case.
The current mental health care system in the US continues to struggle with providing adequate care and services to all that require it due to limited resources, biases from both other professions and the public, and the complexities of treatment of many of those individuals or populations that suffer from mental illness. Mental health nurses, also referred to as psychiatric nurses, are impacted by those same biases, limited resources, and complexities in their role. Stanford psychiatry professor Keith Humphreys says, many of professionals in this field set up a private practice, where they can charge $400 an hour in cash to help people who Humphreys calls “the worried well”, people who enjoy the self-exploration of therapy but don’t necessarily have a mental health problem. “That very well-trained person should be taking care of very, very troubled people. When they don’t, everyone who needs that care loses out.”
Treating high-functioning professionals in a private office is a lot less stressful than doing rounds on a psychiatric ward in a public hospital. Mental health professionals don’t need to participate in the health care system or take insurance to keep their schedules full making it harder and harder for people of lower income to find a therapist. That’s why you can have a lot of mental health professionals in an area, but still have a shortage of care for people in need. The person who’s hurt is the person who’s suicidal, maybe they’re horribly addicted to OxyContin or their child is showing signs of bipolar disorder, and they can’t find somebody to take their insurance. It’s unjust.

Scenario 2: It has been stated that the fields of psychiatry and psychiatry nursing lost interest in the seriously mentally ill (SMI)[ i.e. patients with schizophrenia, PTSD, major depression, bipolar, etc.], and became more interested in working with the worried well. Do you think this is still true in psychiatric nursing? support your answer (Boyd & Luebbert, 2020; Keltner & Steele, 2019)?
After the closures of many of the state Mental health institutions, many people with chronic, severe mental illnesses (SMI) such as schizophrenia, PTSD, bipolar, or major depression after deinstitutionalization occurred were discharged into the community with the hope of being able to live successfully in the community, unfortunately many SMIs are homeless or incarcerated in our overcrowded correctional system. Currently providing humane care is a priority in treating a person with SMI in hospitals and this is still true in psychiatric nursing. In addition, problems are a present due to apathy and public resistance to community mental health treatment for SMI. There is a strong issue of disparity of health care with the SMI and still is not considered a high priority and the problem continues to grow related to the current rise of unemployment and an increase in the the severity of mental health conditions, resulting in a lacking of financial resources to meet the challenges of providing any adequate care such as housing, medical and psychiatric care, social services, and vocational rehabilitation (Lamb & Weinberger, 2017).
As insurance coverage for people with the diagnosis of a serious mental illness (SMI) may be refused due to a pre-existing condition which causes a disparity of health care. Although hospitals are becoming the essential provider for the SMI but assess to these services may be affected because the psychiatric and medical services they need are unprofitable services (Shen, Cochran & Moseley, 2008).
